Please refer to the Terms and Conditions of the individual award for detailed information on renewal application and final reporting requirements.

Funding Agency

Forms and Instructions

CFF Non-Competing Renewal Application
Must be submitted 90 days before the
start of the next budget period.
CFF Final Scientific Report
Must be submitted within 90 days after the project completion.
Student Traineeship Final Summary Report
Must be submitted within 90 days after the project completion.

Submission Guidelines

All Renewal Applications and Final Reports must be submitted via e-mail to grants@cff.org, along with signed
Face Pages (in PDF).

CFFT Non-Competing Renewal Application
Must be submitted 90 days before the start of the next budget period.
CFFT Final Scientific Report
Must be submitted within 90 days after the project completion.
Student Traineeship Final Summary Report
Must be submitted within 90 days after the project completion.

All Renewal Applications and Final Reports must be submitted via e-mail to CFFTAwards@cfftherapeutics.org,
along with signed Face Pages (in PDF).

CF Care Center Financial Report
Must be signed by an authorized Financial Officer, as well as by the Center Director.
Report of Expenditures Form
Must be signed by an authorized Financial Officer, as well as by the Principal Investigator.
Carry-Forward Request Form
For multi-year grants where unexpended funds remain between budget periods.
No-Cost Extension Request Form
In the event the research is not completed within the project period, the PI may request a no-cost extension.
