Scientific and Financial Reporting forms for the Cystic Fibrosis Foundation (CFF)— Please note that the "Reports for CFF" should be used with the following grant types:

Report of Expenditures for CFF— Must be submitted within 90 days after the end of each budget period. The Report of Expenditures also should be signed by the responsible institutional financial officer, as well as the principal investigator.*
Renewal Guidelines & Application for CFF— For awards of two years or more, the final quarterly payment for each budget period will be released upon receipt of a Renewal Application (due the first day of the 10th month of the budget period).*
Final Scientific Report for CFF—For the final year of an award, it is expected that a Final Scientific Report be submitted within 90 days of completion of the project.*
* Failure to comply with this policy will result in forfeiture of any outstanding payments from the CF Foundation
without prior notification.
