For the Cystic Fibrosis Foundation

Please refer to the Terms and Conditions of the inidividual award for further information on reporting requirements. The reports in this section should be used for grants awarded by the CFF including:

Reporting Forms

• Renewal Progress Report must be submitted 90 days before the start of the next budget period.
• Final Scientific Report must be submitted within 90 days after the project completion.
• Final Report for QI Grants must be submitted within 90 days after the project completion.
• Final Summary Report for Student Traineeships must be submitted within 90 days after the project completion.

Financial Reports

Financial reports must be submitted within 90 days after the close of each budget period. Reports must be signed by the authorized financial officer for the Grantee Institution, as well as by the Principal Investigator.

• For Care Center Grants
• For all other CFF Grants

Report Submission

All reports must be submitted via e-mail to grants@cff.org. If electronic submission of the signed report is not possible, the original must be sent to:

Reporting Forms

• Final Scientific Report must be submitted within 90 days after the project completion.
• Final Report for TDN Committee Chairs must be submitted within 90 days after the project completion.
• Final Summary Report for Student Traineeships must be submitted within 90 days after the project completion.
