You will be prompted to electronically sign consent at the end of the Compass Online Service Request form to allow the case manager to work on behalf of the person with CF. At this time, only individuals with CF over the age of 18 and parents, or legal guardians may complete this form. If you are a healthcare provider, a spouse of a person with CF, or other caretaker for a person with CF and would like assistance, please call us.
You may need some, or all, of the following documents to complete the form:
- Insurance card(s)
- Provider information
- List of medications and devices, including name, dosage, and frequency
Estimated annual household income and household size
Supporting documents such as insurance plan details, list of covered drugs, or copy of bills
It will take you less than 10 minutes to fill out and submit your request through this online portal.
You have 60 minutes to submit the form before the form will time out, and all unsaved data will be lost.
